The official way:
sah bum nim: instructor (or "master")
sah boo nim : more intimate and respectful form of "sah bum nim"; literally "teaching father"
In our Taekwondo classes though, we never call the teacher this (not that we chose not to, but because it wasn't what the teachers wanted).
Instead we call our teachers: Mister (up to 4th Dan)
and: Master (5th Dan) - we don't have anyone higher than 5th.

-Boo Sah bum nim: Teacher assistant (from 1st to 3rd degree black belt)
-Sah bum nim: Teacher or instructor (from 4th to 6th degree black belt)
-Sah Gyon nim: Master instructor ( In ITF a Junior Master is a 7th degree black belt and a Senior Master is a 8th degree black belt)
-SanSoon nim: Grand Master (IX Degree - the highest grade)

All of our instructors at our school are addressed Sir or Ma'am
Our teacher who trains us, we call Master as he is a fifth dan
We called our instructor 'Sir' or Mr Farrell.
Anyone 5th dan and over was Master (followed by their surname)
There was good Etiquette in the class, everybody shook everyone's hand at the start and end of every class and everyone called each other Sir or Ma'm.
I liked it, everyone was shown respect from white belt onwards.
We are very informal, first names within the class, Titles when doing official line ups etc
